Nurse's Note: Incoming 9th Grade

For parents of incoming 9th graders

Immunizations: Recent laws have changed the immunization requirements for your child to attend school. The following are the current requirements for attendance at a California school:

  • Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) — 5 doses
    (4 doses OK if one was given on or after 4th birthday. 3 doses OK if one was given on or after 7th birthday.)
    For 7th-12th graders, at least 1 dose of pertussis-containing vaccine is required on or after 7th birthday.
  • Polio (OPV or IPV) — 4 doses
    (3 doses OK if one was given on or after 4th birthday)
  • Hepatitis B — 3 doses
  • Measles, Mumps, and Rubella (MMR) — 2 doses
    (Both given on or after 1st birthday)
  • Varicella (Chickenpox) — 2 doses

As of January 1, 2021, medical exemptions for vaccines may only be issued through the California Immunization Registry – Medical Exemption website (CAIR-MEby physicians licensed in CaliforniaThe parent / guardian must initiate the process. For details, see https://www.shotsforschool.org/laws/exemptions/


Medical 
Management: If your child has a medical condition or physical limitation for which school accommodations need to be made or needs to take medication at school, please ask your child’s California licensed provider / prescriber for the necessary documentation for our health staff to assist your child. We provide forms if the provider prefers to fill out a form. See links on this page. Note: new orders are needed for each school year.

Please provide the forms before the first day of school along with any medication or health device your child may need at school.
